Forms and Policies

Your Visit

We ask that you bring a copy of your insurance card and a photo ID to every visit. If you have previous records you may bring them with you or have them sent to our office. New patients should arrive at least 15 minutes prior to your appointment time to complete the registration process.

Your request will be processed within 24 hours, during normal business hours. If you do not receive confirmation of your request within 48 hours, please contact our office by phone.

Appointments

Appointments can be made by calling our office during normal business hours.

After Hours & Emergency Care

If you are experiencing a life-threatening emergency, dial 911 or go to the nearest emergency room. For all other issues, our automated system will allow you to leave a message for the midwife or physician on call 24-hours a day, 7-days a week.

Prescription Renewals

Prescriptions are renewed during normal office hours. Please give 24 hours notice for all prescription requests. In many instances, we will require that a patient be seen in our office prior to medication renewal.

Financial

All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards, and Care Credit. Please contact our billing office should you have any financial questions or concerns.

Due to the threat of the COVID-19 (Coronavirus), we are taking precautionary measures to protect our staff and patients. We are implementing a temporary NO VISITOR policy at this time. Only patients or patients and their guardians will be allowed into the office. Please note, this is not to inconvenience anyone! The virus is a very real threat and it is essential that we stay healthy in order to provide needed care for our patients. If you are sick, we ask that you CALL the office to speak with a nurse before you come in. The nurse will provide specific instructions for you. Please DO NOT enter the office if you are sick (upper respiratory symptoms, fever, cough, or flu like symptoms). We are closely monitoring the situation and will update you as circumstances change.
